when doing an amalgam filling, do u ALWAYS use zink oxcide phosphate as an insulator before continuing the amalgam layer?
What about composite fillings? do u also use zink oxcide phosphate?
when drilling a tooth from the enamel, how do u know u have reached the dentine?

zinc phosphate is a cement. zinc oxide-eugenol is what is used as intermediate restoration under deep-seated or large cavities. so, no, it is not always used under amalgam restorations.
zinc-oxide-eugenol should not be used under composite fillings as it hampers polymerization and weakens the composite. composite also will not bond with it. you can use glass ionomer for this. light-cured cavity liners are also available in the market.
you will know you have reached dentin because of it's color. enamel is bluish-white whereas dentin is yellowish. dentin is about 1.5mm. from the enamel surface.
as for the book, sorry, but black was a long time ago and his cavity prep ideas (although good to know for background) have been superceded by the advancement in dental materials.
